miDream is a life insurance policy offering support for various critical risk events. Benefits cover death, loss of a Selected Family Member, total & partial permanent disability, named critical illness diagnosis, or hospitalization (more than two nights). miDream includes a savings component for monthly or any additional contributions, allowing for withdrawals from your savings account at any time.

Definitions
Active Policy: The policy remains valid when premiums are paid on time. It ends when a claim is paid after the death of the main policyholder or when the policy lapses.

Beneficiary: The person designated to receive the death benefit, generally a family member.

Lapse: The cessation of insurance benefits due to non-payment of premiums.

Policy: The contract between you and miLife Insurance where, in return for regular premiums, a lump-sum benefit is provided to you or your beneficiary upon an insured event.

Policyholder/Subscriber: The individual holding the insurance policy.

Premium: The monthly amount paid for insurance coverage.

Sum Assured: The guaranteed benefit amount provided if the insured event occurs.
Eligibility
Eligible subscribers are aged 18–59, with Selected Family Members aged 18–74. Both parties must be in good health and reside in Ghana at the time of purchase.
Risks Covered
a. Death: Includes the passing of the policyholder or selected family member.

b. Total and Permanent Disability: A state where the policyholder cannot work for three months and is deemed unlikely to work in any suitable occupation.

c. Partial Permanent Disability: Includes permanent incapacitation due to: Loss of one limb, one eye, hearing in both ears, speech, or partial paralysis.

d. Critical Illnesses: Coverage includes Heart Attack, Cancer, Stroke, Kidney Failure, Major Organ Transplant, Coronary Artery Bypass, Paraplegia, Blindness, Heart Valve Replacement, Aorta Surgery, Major Burns, End Stage Lung Failure, Multiple Sclerosis and Coma.

e. Hospitalization: Benefits are available if hospitalized for over two nights, up to 20 nights.
Premium Payment
Monthly premiums will be deducted from your AT Money Cash wallet automatically. Subscribers confirm payment with their wallet PIN, which will authorize a recurring auto-debit from your ATMoney wallet.
Annual Inflation Adjustor
An annual 10% increase in the risk premium will protect your benefits against inflation, with 7.5% benefits increase applied every February.
Policy Lapse
Policies lapse if premiums are missed for three consecutive months. During a lapse, no claims will be honored. Savings may still be withdrawn.
Reinstatement
Policies can be reinstated with one month's premium after the lapse of the policy. After reinstatement, claims are only accepted for events occurring after six months (180 days) waiting period.
Policy Changes
Selected Family Member can be changed at any time. Policy benefits changes only take effect after six months. During this waiting period, you remain insured under your previous benefit level except for reduction in sum assured which takes effects immediately.
Policy Upgrades/Downgrades
You can upgrade your policy at any time to a higher level or sum assured. The corresponding premium will be deducted for the upgrade. A waiting period of 6 months (180 days) is applicable for the new sum assured. Should an event occur before the waiting period is up, the old sum assured will be paid. You may also downgrade your policy to a lower sum assured. No waiting period applies in this case.
Waiting Period
There is a six-month waiting period for benefits of the policyholder and the Selected Family Member, except for death or disability due to accidents. The waiting period for Selected Family Members begins when they are added to the policy.
Critical Illness
What are the named critical illnesses covered under this policy? i. Heart attack - Heart attack is the death of a portion of the heart muscle as a result of abrupt interruption of adequate blood supply to the area. The diagnosis should be based upon all the following criteria: · A history of typical chest pain · New electrocardiographic changes · An elevation in cardiac enzyme levels.

ii. Cancer - Cancer is the presence of uncontrolled growth and spread of malignant cells and invasion of tissue. Incontrovertible evidence of the invasion of tissue or definite histology of a malignant growth must be produced. The term “cancer” also includes leukaemia, lymphomas and Hodgkin’s disease. Excluded are non-invasive carcinomas in situ, any skin cancer except malignant melanomas, localized non-invasive tumors showing only early malignant changes and tumors in presence of any Human Immunodeficiency virus.

iii. Stroke - Any cerebrovascular incident (or accident) producing neurological sequelae lasting more than 24 hours including: · Infarction of brain tissue · Haemorrhage from an inter-cranial vessel and · Embolization from an extra-cranial source. · Evidence of permanent neurological deficit must be produced.

iv. Kidney failure - End stage renal failure due to chronic irreversible failure of both kidneys to function. This must be evidence by life assured undergoing regular renal dialysis or having had renal transplantation.

v. Major organ - The actual undergoing of a transplant of heart, heart and lung, liver or bone marrow as a recipient.

vi. Coronary artery - Open-heart surgery to correct narrowing or blockage of two or Bypass surgery more coronary arteries by the use of saphenous vein grafts or internal mammary grafting but excluding all non-surgical procedures such as balloon angioplasty or laser techniques. Angiographic evidence of the underlying disease must be provided.

vii. Paraplegia/Paralysis - The complete and permanent loss of use of two or more limbs through paralysis.

viii. Blindness - The total and irrecoverable loss of sight of both eyes due to traumatic injury or disease. The diagnosis must be clinically confirmed by an appropriate consultant.

ix. Heart Valve Replacement - The replacement of one or more valves due to stenosis or incompetence, or combination of these conditions.

x. Surgery of the Aorta The undergoing of surgery to correct any narrowing, dissection or aneurysm of the thoracic or abdominal aorta.

xi. Major Burns - Third degree burns covering at least 20% of the body surface

xii. End Stage Lung Failure: Includes chronic obstructive pulmonary disease (COPD) or emphysema, Scarring of the lungs (pulmonary fibrosis), cystic fibrosis (CF), and pulmonary vascular disease (primary pulmonary hypertension).

xiii. Multiple Sclerosis: With evidence of the typical symptoms of demyelination, persisting neurological abnormalities and impairment function.

xiv. Coma: State of unconsciousness with no reaction to external stimuli or internal needs persisting continuously with the use of life support systems for a period of at least 96 hours resulting in a neurological deficit of a permanent nature.
Claims
For claims related to hospitalization, disability, or death, the benefit is paid to the policyholder or named beneficiary. Required documentation includes: · Completed claim form · Valid national IDs of claimant and deceased Selected Family Member · Proof of death: Medical Certificate, Mortuary Documents, Police report for accidents · Other documentation as may be required

Proof for disability and critical illness should be a medical report/certificate certified by an accredited medical practitioner

Proof of hospitalization from an accredited hospital and certified by an accredited medical practitioner, such as admission notes, discharge notice or hospital invoice.
Maximum Benefit
The maximum benefit payable for a covered life is two times the maximum sum assured of the policy (e.g. GHS 33,200), regardless of multiple subscriptions.
Policy Cancellation
You may cancel your policy anytime. If cancelled within 15 days of starting the policy, the first premium is refunded. There is no cash value for terminating the cover after this period. Upon cancellation, coverage ends, and any savings will be returned to you.
